Employer Paid Group Life and Accidental Death & Dismemberment (AD&D Insurance)

You can never predict when an untimely death will occur; however, proper planning can reduce the financial burden on loved ones. Mutual of Omaha administers these plans, which are designed to provide financial protection to your beneficiaries in the event of your death.

Long-Term Disability

Long-Term Disability works to keep part of your paycheck coming if you become disabled for more than 13 weeks. You are considered disabled if you are prevented from performing at least one of the material duties of your regular job due to injury or illness. The disability plan is administered by Mutual of Omaha.

Preexisting Condition Limitation Disabilities that occur during the first 12 months of coverage caused by a condition which occurred during the three months before coverage begins are excluded.


Definitions

Earnings — based on your gross annual salary in effect prior to the onset of disability. Weekly earnings for hourly employees are based on your average hourly rate of pay in effect prior to the onset of disability. These earnings are used to determine your benefit in the event of claim.

Your disability payments will be combined with, and offset by, any other disability income you receive (Social Security, state disability, etc.) so that your monthly payments total your elected percentage of your “basic earnings.”
